Demand Side Platforms

I know a couple of people on the board here have made quite a bit of money doing Media Buys. I've got a couple of questions for you:

1. What AdServer do you use ( Doubleclick, OpenX, etc.)?
2. What Demand Side Platform do you use (MediaMath, AppNexus, etc.)?
3. In general, how do you use these buys to push affiliate traffic? Do you build a list in a particular vertical, then hit the list with offers? Do you ever just direct link offers?
