How to Do this Contact Form?

Just came across a contact form (at the bottom of this page)

using Contact Form 7 plugin it looks like, where the visitor is given a number of options, depending on their need.
I would like to do the same but haven't yet been able to suss how it's done.
Any ideas please?

Hmmm.

He's using this theme: http://mythemepreviews.com/theme_switcher/?item=squarecode_wordpress
which may or may not include the jquery being used to expand and contract the three different contact forms. But he's definitely built 3 different forms using Contact Form 7 and included them in three different places. You should be able to do that in the Contact Form 7 Dashboard. The jquery is probably not that hard to work up if you search on Google for something like "jQuery expand and contract to hide content" or something along those lines. I'm betting there's enough out there that it can be a copy and paste procedure for you for the most part.
