They can. Avoid using spam words and invite your subscribers to add you to their address book (contacts) after they submit their email (on the thank you page).

It depends. If there's a lot of segmentation taking place then you can sort out which leads are high-quality and which leads are low-quality. With this said, the general consensus is that double opt-in leads to higher quality leads, but this can be different for certain niches (some people simply don't like having to confirm their address- they're lazy).

Going from promotions to the main inbox is a matter of "trust" that you are assigned.
Email deliverability has a lot of variables to it, but one that I've found to be important is open rates. This is something you can optimize for.
Segment out your emails to send to the people that most frequently open your stuff. And also do what DanielS said.
Be ruthless - if there are subscribers that aren't opening your emails at all, remove them from your lists. Your open rate will increase without the dead weight dragging it down.
