Wordpress - Custom Form That Stores Data Into Database UI

Just looking for suggestions on a flow I'd like to execute. Probably some plugins out there that can do exactly this. I hope, anyway.

Ideally this is the flow I want:

1. User selects the pricing package they want.
2. Based on their selection we show them a custom form & they fill it out.
3. We use the form to gather & populate a database we can manage/view on the backend (viewer friendly preferred).

Any ideas for wordpress plugins or combos of wordpress plugins that'd allow this to happen? Alternative I assume is to build something custom? This feels just like a simple issue that I just need someone to point me in the right direction on...

Skills I have: Basic html/css/wordpress knowledge.
 
Check out Gravity Forms. It has a ton of extra plugins too, I'm fairly certain it can do everything you want. I'm not sure about populating a database, but it can save your filled out forms in the Wordpress database and you can sort through them in the dashboard.
 
Gravity forms and/or ACF. I've built stuff like this many times.
